Schedule a Pelvic Floor Evaluation
Once you decide you would like to schedule an evaluation, our front office staff will be able to make that experience easy for you. When you call the main number (419-893-7134; option 5), the front office staff will get all of the important information from you in terms of verifying your insurance and discussing the process of getting a referral for physical therapy services.
Information you will need to have ready is:
- Name, address and phone number
- Insurance information (have card ready)
- The doctor who is referring you to us
- The referring doctor’s phone number
- The type of evaluation you will need (pelvic pain, bladder control issues, frequency, bowel control issues, sexual dysfunction)
